Self-Direction PPP Support Specialist

Horizon Blue Cross Blue Shield of New Jersey is seeking a Self-Direction PPP Support Coordinator to conduct quarterly in-person evaluations for Personal Preference Program members. You will manage budgeting, backup plans, capacity forecasting, and safety planning to ensure the cash management plan meets the member’s needs.
You will review monthly cash expenditures, update plans quarterly, maintain the member’s electronic medical record, and provide clear communication.
