Job Title Social Worker Job Summary We are seeking a dedicated and compassionate Social Worker for a contract assignment in Savannah, Georgia. This role is integral to providing comprehensive support to patients and their families within a dynamic healthcare setting. The Social Worker will be responsible for conducting psychosocial assessments, developing intervention plans, and facilitating access to essential resources. You will play a crucial role in advocating for patient well‑being and ensuring continuity of care. This is an excellent opportunity for an experienced Social Worker to contribute to a patient‑centered environment and make a meaningful impact on individuals' lives. Key Responsibilities Conduct comprehensive psychosocial assessments to identify patient and family needs, strengths, and coping mechanisms. Develop and implement individualized care plans, including discharge planning, crisis intervention, and grief counseling. Provide emotional support, counseling, and education to patients and families regarding illness, treatment, and recovery. Facilitate access to appropriate community resources, support groups, and long‑term care options. Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure integrated and holistic patient care. Advocate for patients' rights, preferences, and access to necessary services. Maintain accurate and timely documentation of all patient interactions, assessments, and interventions in compliance with facility and regulatory standards. Required Qualifications Minimum of 1-2 years of recent experience as a Social Worker. Preferred Qualifications Not specified.
