Location & Schedule

Location: Los Angeles, CA (Koreatown, Aroma Building)

Work schedule: Full-time, Monday through Friday, 9:00 AM – 6:00 PM

Compensation: $23 – $28 per hour, depending on qualifications, experience, and position level

Responsibilities

Monitor health data for patients with chronic conditions such as hypertension and diabetes and follow up on routine alerts.

Monitor patient-generated health data, conduct patient outreach and support calls, and maintain accurate documentation in compliance with HIPAA requirements.

Coordinate and communicate effectively with clinic staff and healthcare providers.

Visit prospective clinics to introduce RPM and CCM programs, develop collaborative relationships, and identify new business opportunities.

Requirements

Minimum one (1) year of clinic experience or care coordination experience (MA certification)

Strong communication and documentation skills

Computer proficiency, including Email, Microsoft Word, and Excel

Bilingual in Korean and English

Authorized to work in the United States

Preferred Qualifications

Experience with RPM programs and/or Medicare-related services

Experience using EMR/EHR systems
