245D Client Services & Intake Lead

Hearts In Homes MN is seeking an experienced Minnesota 245D professional to lead client services and intake. This hands-on role manages referrals from first contact through start of care, coordinating with county case managers, families, and our scheduling support.
You will own the client journey, manage documentation, caregiver matching, and ongoing communication, while collaborating with a virtual assistant and recruiting to ensure smooth starts and compliant records across Minnesota.
