Healthcare Field Sales: Territory Account Manager

VitalCaring Group is seeking a Field-Based Account Manager to drive referral growth and admissions within an assigned territory. You will build relationships with physicians, hospitals, SNFs, ALFs, and other partners, expanding the referral network and consistently meeting targets.
You will travel daily for in-person visits, managing outreach, follow-ups, and CRM updates while collaborating with clinical and operational teams to support patient transitions and service delivery.
