Position Overview:
The Health Promotion Case Manager assists in developing and implementing programs aimed at improving the health outcomes of refugee populations. This position collaborates with local health departments, community organizations, and refugee service providers to promote health education, improve access to healthcare services, and connect refugees and other individuals eligible under the Office of Refugee Resettlement with culturally appropriate health resources in the Tucson area.
Key Responsibilities:
Health Promotion:
- Design and implement health promotion programs targeting refugee populations, focusing on prevention, education, and access to healthcare.
- Collaborate with healthcare providers, social service agencies, and community organizations to enhance service delivery to refugees.
- Coordinate health education workshops, community outreach events, and other activities aimed at promoting wellness.
- Provide culturally competent health education to refugee individuals and families on topics such as nutrition, chronic disease management, mental health, hygiene, and preventive care.
- Develop educational materials in multiple to address the diverse needs of the refugee population.
- Refer individuals to community health screenings, vaccination drives, and health-related events.
Case Management and Advocacy:
- Assist refugees in navigating the healthcare system, including scheduling medical appointments, transportation, and interpreter services.
- Advocate for refugees’ health needs at local healthcare providers and ensure that care is culturally and linguistically appropriate.
- Monitor and follow up with refugees to ensure they are receiving necessary health services and support.
- Collect and maintain data on program outcomes, service delivery, and client needs.
Collaboration and Networking:
- Build and maintain relationships with community partners, healthcare institutions, and governmental agencies.
- Attend community meetings, health fairs, and other networking events to represent the organization and advocate for refugee health concerns.
- Coordinate with mental health professionals, social workers, and other service providers to ensure a holistic approach to refugee health.
Qualifications:
- High School diploma or GED required
- Bachelor’s degree in social work, sociology, psychology, or a related field
- Case Management experience
- Experience in health promotion/administration, preferably with a diverse/refugee population
- Provide and maintain a valid Level One Fingerprint Clearance Card, clear the Central Registry Background Check and a criminal background and motor vehicle screening; must maintain valid AZ Driver’s License, a clear driving record and reliable transportation
- Ability to work evenings or weekends when necessary

Other Knowledge, Skills, and Abilities:
- Proficient computer skills and knowledge of Outlook and Microsoft Office Programs (i.e. Word, Excel, PowerPoint)
- Ability to speak, read, and write English
- Ability to speak, read, and write in another commonly used by our client population
- Excellent oral and written communications skills
- Strong interpersonal skills and ability to handle sensitive and confidential situations
- Ability to relate well with families of varied lifestyles, backgrounds and ethnicities
- Experience delivering, designing, and/or developing training presentations, preferably in a multicultural context
Physical Demands:
- While performing the duties of this job, the employee may be required to sit or stand for long periods.
- The employee frequently is required to use hands to hold objects and operate office equipment.
- The employee may be required to occasionally lift and/or move 25 pounds.
