As a Vascular Nurse Clinician, you'll play a vital role in independently coordinating and facilitating patient care between multiple disciplines within UI Health Care and the local communities. Nurse Clinicians facilitate the continuum of care across our main campus, Iowa River Landing, and other campus locations. They provide nurse triage for patient conditions and referrals, work closely with patients over the long term, build meaningful relationships, and support recovery and wellness journeys. The Vascular Nurse Clinician will design and implement health information for patients and families.

Our clinic operates Monday–Friday and is closed nights, weekends, and holidays. Hours are flexible with 8- and 10-hour shifts available; schedules can be discussed through the hiring process.

Position Responsibilities

  • Collaborates with nursing, medical staff, and other health professionals in the development and review of standards and in planning, implementing, and evaluating patient care for the vascular patient population. Provides direct nursing care when necessary. Follows universal precautions.
  • Oversees patient flow through the Heart and Vascular Clinic.
  • Independently coordinates and facilitates patient care between multiple disciplines within UIHC and the community (including home health agencies, referring physicians, and other community support services). Facilitates continuity of care.
  • Coordinates patient care to better utilize supportive services through patient visits, pre-scheduled testing, follow-up on test results with patients and physicians, obtaining medical history, coordinating follow-up appointments, providing medication and disease process teaching, etc.
  • Participates in the ongoing management of the patient to facilitate timely care.
  • Facilitates care of vascular patients requiring admission, coordinating admission procedures and pre/post-procedure care.
  • Acts as a clinical resource or consultant for vascular patient populations.
  • Participates in implementation and requirements of managed care programs for vascular patient populations.
  • Operates autonomously in communicating with health care team members and directing patient care across settings.
  • Communicates autonomously with referring physicians and health care facilities.
  • Revises patient care plans via telephone within established protocols and performs telephone triage of calls to the vascular service, facilitating medication refills as needed.
  • Provides cross-coverage to other UI Heart & Vascular Center Nurse Coordinators PRN.
  • Performs on-call, clinical duties, special projects, and other tasks as needed.
  • Participates in the ongoing implementation of current regulatory standards to meet state and national agency accreditation.
  • Designs and implements patient/family health education activities for vascular patients, recognizing individualized needs.
  • Provides patient education in vascular clinics, including assessment of learning needs post-operatively and education related to diagnosis, procedures, risk factor modification, and treatment regimens.
  • Facilitates learning and provides consultation to external agencies, communities, and other health care professionals.
  • Incorporates roles as patient care provider, advocate, consultant, educator, researcher, and quality manager.
  • Participates in the development and implementation of division/unit quality assessment and improvement programs using appropriate risk and quality management data.
  • Uses relevant nursing research to improve clinical practice and investigate nursing care problems related to a specific patient population.
  • Follow general policies and procedures in accordance with the regulations of the University of Iowa Hospitals and Clinics and the UI Heart and Vascular Center.
  • Other duties as assigned.

Education Requirement

  • Baccalaureate Degree in Nursing is required.

Experience Requirement

  • Minimum 1 year providing direct nursing care or management/supervision of cardiovascular medical/surgical patient population required.
  • Ability to work autonomously in a high-volume ambulatory setting with limited supervision.
  • Excellent written and verbal communication skills required.
  • Demonstrated knowledge and clinical expertise in a specific clinical area of practice required.

Certification/Licensure Requirement

  • Current certification in BLS required by start date.
  • Current license to practice nursing in the State of Iowa required by start date.

Desired Qualifications

  • 3 years providing direct nursing care or management/supervision of cardiovascular medical/surgical patient population.
  • 1–3 years of previous knowledge and experience providing wound care.
  • Strong organization and time management skills and documented excellent interpersonal skills with patients, families and the health care team.
  • Demonstrated ability to professionally interact with patients/families, nursing staff, and other multidisciplinary team members required.
  • Current job-related professional certification.
