Role Overview

The Medical Social Consultant provides comprehensive psychosocial assessment and discharge planning for oncology, epilepsy, and related patient populations. The consultant collaborates with interdisciplinary teams, supports patients and families through complex decision‑making, and utilizes community resources to facilitate safe and effective transitions of care.

Duties & Responsibilities

  • Conduct comprehensive psychosocial assessment of patients and families, addressing emotional, cultural, financial, and environmental factors.
  • Develop and document individualized intervention plans consistent with interdisciplinary goals.
  • Coordinate and advocate for community resources, financial assistance, and surrogate decision‑making processes.
  • Facilitate discharge planning, ensuring timely communication of care plans to physicians, interdisciplinary teams, patients, and designated caregivers.
  • Attend daily Multidisciplinary Rounds and unit/clinic–based rounds to collaborate on patient care.
  • Identify and resolve barriers to discharge and participate in conflict resolution to expedite transition of care.
  • Provide supportive care for patients considered for hospice/palliative care and lead family meetings to discuss goals of care.
  • Lead educational and group interventions for patients and families, addressing medical, psychosocial, and spiritual needs.
  • Maintain competency for Licensed Social Worker (LSW) and Licensed Clinical Social Worker (LCSW) credentialing within Illinois.

Minimum Qualifications Required

  • Master’s degree in social work, behavioral science, or a related health specialty from an accredited institution.
  • Current Illinois licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW).
