Duties & Responsibilities
- Perform comprehensive psychosocial assessments for patients/families, addressing social, emotional, cultural, mental status, environmental, and financial circumstances and collaborating with interdisciplinary teams.
- Document initial psychosocial assessments, interventions, and transitions in a timely, clear, and legible manner.
- Manage and resolve barriers to discharge planning, coordinate placement and resource utilization, and facilitate timely resolution of system problems.
- Facilitate health care surrogacy and/or guardianship processes for patients deemed non‑decisional, leading efforts to identify family and surrogate decision makers.
- Collaborate with legal and complex service line departments to file for Illinois state guardians when necessary.
- Coordinate groups for supportive interventions or educational opportunities and address financial issues related to insurance coverage and payment.
- Conflict resolution and management of complex family dynamics impacting plan of care and discharge planning, including coordination of complex clinical case conferences.
- Investigate, manage, and report suspected abuse or neglect of minor children and vulnerable adults, providing safety planning and documenting cases of suspected intimate partner violence/domestic violence of adults.
- Assess depression and suicidal ideation using validated tools, addressing psychiatric symptoms and substance use disorders.
- Provide supportive care for patients considered for hospice/palliative care and participate in family meetings to discuss goals of care and end‑of‑life planning.
- Address needs for patients who are unhoused and perform assessments of the physical environment to prevent crises.
- Participate in quality improvement committees and related activities where Social Work involvement is warranted.
- Provide cross‑coverage to other clinics and inpatient units as assigned and assist with training new staff and student interns.
- Collect and report financial indicators related to length of stay, avoidable days, resource utilization, discharge barriers, readmission rates, and denials.
- Use data to drive decisions and plan/implement performance improvement strategies related to social work for assigned patients and units.
- Assume responsibility for professional development and Social Work clinical education requirements by participating in workshops, conferences, and in‑services.
- Develop and lead groups or education sessions based on program needs.
- Perform inpatient and ambulatory medical social work duties and manage discharge planning through placement coordination and resource utilization.
- Maintain competency for Licensed Social Worker (LSW)/Clinical Licensure (LCSW) and program credentialing to allow billing of services.
Minimum Qualifications Required
- Master's degree in social or behavioral science or a related health specialty area from an accredited institution.
- Current state licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Illinois.
