Job Description

As the home health community liaison, the primary responsibility is to develop and maintain relationships with referral sources such as physician hospitals, skilled nursing facilities, assisted living communities, and other health care providers. The role focuses on building long‑term partnerships with the community. Primary Responsibility

Serving as a liaison between referral sources and the clinical team to ensure seamless communication and quality patient care. Implementing strategic marketing plans to generate new referrals and growth including patient outreach. Establishing new relationships with referral sources such as physician officers, case managers, discharge planners, and social workers. Maintaining new relationships by conducting regular visits, presenting services, and educating potential referral sources about home health services. Working with the clinical team and administrative team to develop a seamless transition of patients from facility to home. Representing the agency at community events, health fairs, and professional networking meetings and collaborating with the clinical team to ensure high quality service and patient satisfaction. Maintaining compliance with all state and federal regulations related to marketing, patient privacy, HIPAA, and ethical regulations. Preferred Experience

Previous experience with Medicare‑Certified home health, hospice, or home care is preferred, with an existing network of healthcare contacts in the local community. Compensation
