- Demonstrates standards of performance (ownership, teamwork, communication, compassion) that supports principles of service excellence
- Communicates with internal and external stakeholders using problem-solving skills to facilitate patient placement and transfers into and out of Cape Fear Valley Health System
- Receives physician orders on new admissions/transfers
- Monitors bed availability and coordinates bed placement, transfers, and discharges
- Maintains a list of patients awaiting transfer into Cape Fear Valley Health System
- Serves as a liaison for patients, families, physicians, and staff related to patient transfers, bed availability and patient flow issues
- Facilitates effective utilization of technology available for expediting patient flow through the health system
- Collaborates with Nursing Leadership, Care Coordination, and individual Units to facilitate patient throughput
- Other duties as assigned
The following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this job:
Education and Formal Training:
- High school diploma or equivalent required
- EMS credential preferred
- Computer experience required
- Hospital and/or business office experience with specific focus in patient logistics preferred
- EPIC experience preferred
- Ability to communicate needs and concerns in a constructive and positive manner.
- Ability to prioritize work and maintain control in stressful situations
- Exhibits creativity and flexibility in problem solving
- Ability to collaborate with a variety of individuals and disciplines
- Ability to communicate and resolve patient concerns
- Visual and auditory acuity required
- Able to articulate clearly and succinctly
- Work requires long periods of standing, sitting, or walking
- Must have command of the English language
- Visual and auditory acuity
