Primary Duties and Responsibilities Under the direction of a registered nurse or physician and under assigned administrative supervision, provides care within the scope of the Vocational Nurse Practice Act and regulatory standards. Assists in the assessment, planning, implementation and evaluation of patient care. Follows patient identification procedures, provides appropriate information for all patient procedures, and collects and handles specimens. Assesses and implements prescribed interventions for the patient.
Implements and documents discharge plan for each patient. Recognizes that ethical, legal and medical dilemmas exist in nursing practice and confers with the appropriate resource personnel when making ethical decisions. Participates in maintaining a safe patient care environment. Assists with the care and maintenance of department equipment. Completes accurate charting of patient records and record‑keeping of equipment and quality measures. Collaborates with other disciplines of the health‑care team to ensure delivery of care across the continuum, providing timely, safe and quality care. Responsible for providing a safe, HIPAA‑compliant environment for the patient and the staff. Performs other duties as assigned.
Primary Care and Urgent Care:
Assist medical personnel with special procedures, minor surgical procedures and/or diagnostic exams. Process in‑office tests, collect specimens, and perform procedures within the scope of the Vocational Nurse Practice Act and regulatory standards.
Qualifications
High School diploma or equivalent.
One or more years of experience in a healthcare facility (Specialty Clinics can hire a new graduate LVN with no experience).
Basic keyboarding skills and knowledge of Microsoft Office Suite, including Outlook, Word and Excel (preferred).
Basic knowledge of medical terminology (preferred).
Experience with Electronic Medical Records (preferred).
Home Health: At least one year of experience in public health or home‑care nursing. Must be a licensed driver with an insured automobile in good working order.
Hospice: Community health/hospice or medical/surgical experience is preferred. Understanding of hospice philosophy and the needs of the terminally ill. Must be a licensed driver with an insured automobile in good working order.
Primary Care and Urgent Care: Completion of educational courses (within six months of hire): New Hire LVN course – Primary Care or Urgent Care.
License Required Current LVN License to practice in the State of California.
License Preferred N/A
Certifications Required Current BLS Certification.
Certifications Preferred Phlebotomy Certification.
