Position Summary

The Transplant Nurse Coordinator (CNCII) is a Registered Nurse responsible for coordination of clinical care for transplant recipients and living donors. The role operates under supervision of transplant surgeons and physicians, ensuring compliance with UNOS, CMS, and UI Health policies throughout the pre‑transplant and living donor phases.

Duties & Responsibilities

  • Evaluate patients at the appropriate UI Health site of care.
  • Coordinate care for transplant recipients from referral until waitlisted, and for living donor patients.
  • Document pre‑transplant patient coordination and education in compliance with UNOS, CMS, and UI Health procedures.
  • Order and review all testing aligned with UNOS and CMS guidelines, collaborating with the provider in a timely manner.
  • Develop, coordinate, and communicate each patient’s plan of care based on ongoing assessment.
  • Collaborate with the multidisciplinary health care team to improve patient outcomes.
  • Perform interventions as prescribed by the provider, accurately documenting in the electronic medical record.
  • Assess, engage, and acquire clinical information to develop an appropriate patient educational plan.
  • Recognize symptoms, establish priorities, and respond to patients’ changing needs throughout the transplant care continuum.
  • Ensure patient addition to the UNOS waiting list and timely listing letter submission per UNOS requirements.
  • Coordinate and schedule all living donor transplant surgeries as assigned.
  • Attend and participate in transplant QAPI, listing, department, multidisciplinary planning, and other meetings as needed.
  • Provide cross-coverage for the transplant data coordinator by entering living donor information in UNOS prior to surgery.
  • Delegate appropriate tasks per the Illinois State Nurse Practice Act and provide support and feedback to nursing and support personnel.
  • Demonstrate commitment to UI Health’s values through teamwork and effective communication to achieve quality outcomes and patient safety.
  • Integrate evidence-based practice and current standards to provide high-quality patient care.
  • Support Shared Leadership and participate in organizational and departmental leadership activities.
  • Serve as a preceptor, mentor, and resource to other nursing staff and students.

Minimum Qualifications

  • High school diploma or equivalent.
  • Graduation from an accredited school of nursing.
  • Eligibility for registration with the Illinois Department of Financial and Professional Regulation (Division of Professional Regulation).
  • Two years of nursing experience in the assigned branch of service, including one year comparable to a Clinical Nursing Consultant I.
  • Effective verbal and written communication in English and Spanish.
  • BSN (or higher) from an accredited nursing program.
  • Current BLS/CPR and other unit-specific certifications.
  • Current advanced cardiac life support; within 6 months of hire for main hospital.
  • Nationally certified through an ANCC-approved certifier in a relevant clinical area; within 6 months of hire.

Preferred Qualifications

  • Previous RN experience working with solid organ transplant patients.
