Delaware Opportunities, Inc. is seeking a full-time Social Care Services Navigator to join our Community Services Department. The ideal candidate is hardworking, reliable, and trustworthy, with the ability to support clients in identifying and addressing unmet health-related social needs.
Qualifications
High school diploma or GED and/or 2 years of experience in Human Services
Valid NYS driver’s license
Strong communication and organizational skills
Attention to detail and a non-judgmental approach
Ability to work independently and as part of a team
Ability to meet with clients in office and community settings, including home visits
Schedule & Pay
35 hours per week
Starting pay: $22.93 per hour
Responsibilities
Conduct screenings to identify unmet health-related social needs
Assist clients with navigating and accessing services
Determine eligibility for additional Delaware Opportunities programs
Make appropriate internal and external referrals
Develop a Social Care Plan for eligible members
Comply with all applicable Codes of Conduct and Conflict of Interest policies for Delaware Opportunities and the Care Compass Collaborative Social Care Network
Participate in required trainings
Maintain accurate documentation and records
Perform related duties as assigned
