General Responsibilities:

Under the supervision of the Supervisor of Residential Services, the Resident Service Coordinator provides individual case management and group programming for senior tenants.

Specific Duties And Responsibilities

Provides monthly in-home assessments of the social and emotional needs of the tenant.

Assists formerly unhoused tenants in transitioning back to the community.

Develops and implements a written case plan to provide a full range of social services for the tenant and their family.

Provides individual and family counseling/guidance to resolve any issues the tenant may face.

Prepares specific information about health, welfare, education, and recreational services available to the tenant, including trainings on tenancy obligations.

Assists tenants in developing informal support networks and facilitates opportunities for tenant engagement and participation in group meetings and recreational activities.

Screens tenants for eligibility and assists with applications for benefits.

Conducts referrals to JASA and other providers for legal aid, protective services, vocational placement, volunteer opportunities, primary care, mental health services, etc.

Educates tenants on service availability, application and benefit procedures, tenant rights, and provides advocacy when appropriate.

Ensures tenants maintain housing subsidies and benefits required to remain in permanent housing.

Evaluates tenant functioning, monitors risk, assesses current services, and updates the case plan during in-home visits.

Provides regular reports to the Supervisor of Residential Services on common problematic themes and patterns among tenants.

Consults with the Supervisor on high-risk tenants, including those at risk for eviction.

Performs other related tasks, special assignments, and duties as assigned.

Requirements

Graduate of an accredited college or university with a bachelor's in Social Work or related field.

Minimum one-year relevant experience.

Computer proficiency including Microsoft Office, Google Drive, and electronic case record system.

Excellent interpersonal, communication, writing, and speaking skills.

Strong organizational and multitasking skills.

Bi-lingual English/Spanish preferred.

Ability to understand and appreciate the role of race, religion, ethnicity, sexual preference, and individual values in service delivery.
