Summary Under general direction, the Habilitation Specialist is responsible for designing and implementing individualized programs for the acquisition of functional skills that lead to increased independent community participation for the individuals served. This role aligns with agency and governmental regulations and includes supervisory responsibilities for Habilitation Assistants.
Essential Duties & Responsibilities
Serve as a liaison between the agency, the people we support, and families; advocate for service users.
Develop programs and day habilitation plans for persons served.
Implement active treatment plans inside and outside the building.
Supervise non‑professional staff in implementation of day habilitation plans.
Participate in individual program planning process.
Orient, train, evaluate, delegate, supervise, and recommend hiring and disciplining of subordinate staff.
Collaborate with Service Coordinators for program development and referrals.
Maintain attendance and required records/paperwork.
Utilize community resources for program implementation and community integration.
Carry out basic administrative tasks related to caseload and assignments.
Plan group and individual activities and assign staff to implement them.
Train and instruct support staff in routine, planning, and general procedures.
Attend supervisory staff meetings.
Prepare and adapt materials and set up furniture and equipment for activities.
Participate in scheduled in‑service training programs.
Provide support/coverage to other classrooms as needed.
Engage in activities outside the activity room when necessary, including trips, pool, gym.
Select and utilize educational materials.
Teach persons served to become more independent.
Drive agency vehicles for transportation, pickups, deliveries, community outings, and medical emergencies as required.
Assist persons served with transportation between buses and buildings.
Perform direct care responsibilities including feeding and toileting as needed.
Initiate precautions within established framework to ensure safe work practices.
Perform other related duties as requested.
Qualifications
Valid New York State Driver’s License (preferred).
Ability to read, write, speak, and understand English.
Good oral and written communication skills.
Good interpersonal skills to interact effectively with coworkers, staff, service users, and families.
Good organizational and teaching skills.
Proficiency with a personal computer and appropriate software.
Ability to operate wheelchairs and adaptive equipment utilized by persons served.
Must maintain confidentiality of the records of the persons served.
Ability to analyze problems and determine corrective measures.
Education and Experience
Bachelor’s Degree in Human Services or related field.
One year of postgraduate experience required.
