Responsibilities

Collaborate with RN case managers, nurses, physicians, insurers, and staff to proactively identify and address patient needs.

Identify and address discharge barriers: financial, emotional, psychosocial, medical, and spiritual.

Maintain thorough knowledge of Medicare, Medicaid, private payer regulations and managed‑care concepts, and implement length‑of‑stay management.

Perform comprehensive assessments and develop discharge plans for high‑risk patients with psychosocial needs.

Coordinate and facilitate meetings between the care team and patients/families/caregivers to support decision‑making while honoring preferences and values.

Intervene for psychosocial support during chronic or terminal illness; provide counseling, advocacy, education, and referrals for emotional, social & financial aspects of care.

Offer emotional support for patients and families during end‑of‑life, terminal illness notifications, death notifications (with physician), and crisis interventions.

Qualifications

Master’s degree in Social Work (MSW).

Minimum 1 year of experience in a Master’s Social Work internship.
