Care Coach 1 – Gadsden, Leon or Wakulla counties, Florida
Base pay range:
$53,700 - $72,600 per year (starting salary $60-65k based on qualifications/experience). Compensation may vary by location and experience.
Role Overview:
The Care Coach 1 evaluates members’ needs and requirements to achieve and/or maintain optimal wellness by guiding members and families toward resources and facilitating interaction with appropriate care services. The role involves meeting members in their homes, assisted‑living facilities, or long‑term care facilities (75‑90% local travel). The coach supports members in accessing the least restrictive setting to maintain well‑being and drives health outcomes.
Responsibilities
Visit Medicaid members in homes, assisted‑living, or long‑term care facilities (75‑90% local travel).
Assess and evaluate member needs and establish a member‑specific care plan.
Ensure members receive services in the least restrictive setting to achieve optimal well‑being.
Plan and implement interventions to meet those needs.
Coordinate services, monitor and evaluate the case‑management plan against personal goals.
Guide members/families toward appropriate resources.
Facilitate interactions with other payer sources, providers, interdisciplinary teams, and other involved stakeholders per contract requirements.
Required Qualifications
Bachelor's degree with at least 2 years of relevant experience (Health Services, Social Work, Psychology).
Registered Nurse (RN) licensed in Florida with at least 2 years of relevant experience.
Licensed Practical Nurse (LPN) licensed in Florida with at least 4 years of relevant experience.
Case Worker with a minimum of 6 years of Health Services/Case Management experience.
Prior experience with Medicare & Medicaid recipients.
Intermediate to advanced computer skills (Microsoft Word, Excel, Outlook).
AHCA fingerprint required.
Screening requirement for Tuberculosis under Humana’s TB program.
Valid state driver’s license and proof of personal vehicle liability insurance (Driver safety program).
Availability to work Monday‑Friday 8:30 am‑5:00 pm.
Preferred Qualifications
Bilingual English/Spanish with ability to pass a Language Proficiency Exam.
Community Health Worker experience.
Prior nursing home diversion or long‑term care case‑management experience.
Experience with electronic case‑note documentation across multiple systems.
Experience working with geriatric population.
Experience in health promotion, coaching and wellness.
Knowledge of community health and social service agencies and additional community resources.
701B Certification.
Medication application (RFA) experience.
