Visit Medicaid members in homes, assisted living, long‑term care facilities, and other care settings – 75‑90% local travel. Assess and evaluate member needs to establish a member‑specific care plan. Ensure members receive services in the least restrictive setting to achieve optimal well‑being. Plan and implement interventions to meet member needs. Coordinate services, monitor and evaluate the case management plan against the member’s personal goals. Guide members/families toward resources appropriate for their care. Facilitate interactions with other payer sources, providers, interdisciplinary teams, and others involved in the member’s care as required by the comprehensive contract. Required Qualifications

Minimum of 2 years relevant experience and a Bachelor’s degree (Health Services, Social Work, Psychology) OR Registered Nurse (RN) licensed in Florida with at least 2 years relevant experience. Licensed Practical Nurse (LPN) licensed in Florida with at least 4 years relevant experience. Case Worker with a minimum of 6 years Health Services/Case Management experience. Prior experience with Medicare & Medicaid recipients. Intermediate to advanced computer skills (Microsoft Word, Excel, Outlook). AHCA Fingerprint required. Tuberculosis (TB) screening program screening required. Valid state driver’s license and proof of personal vehicle liability insurance. Ability to work Monday‑Friday 8:30 am‑5:00 pm. Preferred Qualifications

Bilingual English/Spanish with a Language Proficiency Exam. Community Health Worker experience. Nursing home diversion or long‑term care case management experience. Experience with electronic case note documentation and multiple computer applications/systems. Experience working with geriatric population. Experience with health promotion, coaching and wellness. Knowledge of community health and social service agencies and additional community resources. 701B Certification. Medication application (RFA) experience. Equal Opportunity Employer
