Position Summary The Social Services Designee supports residents and families throughout the admission and discharge process by managing referrals, screenings, tours, approvals, and documentation while also marketing facility services and building relationships with referral sources. This role clarifies legal documents, assists with emotional and psychosocial needs, and helps residents navigate advanced directives, financial aid applications, and transitions within the facility. As part of the interdisciplinary care team, the designee conducts social assessments, mental health screenings, and required MDS/CAA documentation to develop individualized care plans that promote each resident’s highest level of functioning. Additional responsibilities include maintaining accurate records, ensuring regulatory compliance, upholding professional communication and confidentiality standards, participating in quality improvement efforts, and contributing to policy development to support residents’ rights and overall well‑being.
Qualifications
Must recognize and understand the psychosocial and emotional needs of residents.
Experience in healthcare, skilled nursing facility or assisted living community.
Strong skills in communication, organization, management, and leadership.
Ability to perform essential functions of the position with or without reasonable accommodation.
Must qualify for employment after a criminal background study, per guidelines of the Minnesota Department of Human Services.
