Position Summary

The Social Services Designee interacts with residents, families, visitors, facility staff, and community agencies during the admission and discharge process. They market the services offered, establish relationships with referral sources, clarify legal documents, and identify psychosocial strengths and needs of each resident. The role assists in developing individualized plans of care and provides medically-related social services to help residents attain or maintain the highest level of functioning possible. Essential Responsibilities And Duties

Facilitates and coordinates the admission process and discharge planning: accepts admission referrals, obtains necessary information, gives tours, makes referrals for screening, obtains approvals, assists with paperwork, maintains records, assists with financial assistance applications, documents discharge plans, and coordinates home health equipment. Communicates and interacts with other staff, families, residents, community agencies and visitors: assists families with adjustments, accepts placement referrals, acts as liaison, visits residents, informs them of rights, supports grieving, informs of room changes, encourages family involvement, and refers to community resources. Participates in the interdisciplinary patient care planning team: completes PHQ‑9 and BIMS, completes social history, documents follow‑up, completes MDS sections, helps develop care plans, and coordinates conferences. Maintains professional conduct: is courteous, follows dress code, keeps confidentiality, promotes QAPI, maintains records, attends continuing education, and abides by abuse prevention and state/federal certification regulations. Qualifications

Must recognize and understand the psychosocial and emotional needs of residents. Must have strong skills in communication, organization, management, and leadership. Must be able to perform essential functions with or without reasonable accommodation. Must qualify for employment after a criminal background study per Minnesota guidelines. Preferred

Experience in healthcare, skilled nursing facility, or assisted living community. Required Skills And Abilities
