Base pay range $53,000.00/yr - $55,000.00/yr

Duties And Responsibilities

Provide in-home, in-office and/or community assessment of the emotional, social and physical needs of older adults reporting any form of abuse or mistreatment.

The development and implementation of a comprehensive assessment, service plan and safety plan for the provision of the full range of social services to address the abuse experienced by the older adult and if appropriate the alleged abuser.

Complete required web-based case documentation that is in accordance with JASA and funder requirements.

Supportive counseling and guidance in the resolution of problems of the older adult and/or the alleged abuser/family member/caregiver.

Referral to JASA Legal Services for Elder Justice and/or other legal organizations.

The provision of appropriate resources to older adults and collaterals regarding ways of coping and preserving individual functioning and autonomy within the context of the abuse experienced by the older adult

Screening for eligibility in securing and maintaining benefits and entitlements, such as transportation, Home Care, Meals on Wheels, SNAP, Public Assistance/ Case Assistance, Medicare, Medicaid, emergency financial relief, etc.

Referrals to community based organizations.

Identify high risk, crisis, and/or emergency cases.

Conduct group counseling to strengthen and empower the older adult and build community among their peers.

Participate in community presentation, education, and outreach activities.

Participate in internal and external training, workshops, and conferences

Attend weekly individual and group supervision and all other mandatory meetings as assigned.

Perform such and other duties as assigned.

Qualifications

Graduate of an accredited college or university with a Master’s of Social Work (MSW) or equivalent Masters-level degree in Gerontology, Psychology, Human Services, or other Social Services related field.

Exemplary written and verbal communication skills.

Experience in individual counseling, utilizing a variety of treatment modalities, including, but not limited to Motivational Interviewing.

Must be computer literate, including ability to learn and use program-specific software for daily data entries.

Ability to travel to clients' homes using mass transit, and to negotiate any barriers in clients' residences.

Bilingual preferred (Spanish, French-Creole, Mandarin, etc.)

An understanding and appreciation of the roles of race, religion, ethnicity, gender, sexual preference, and individual values as they relate to serving vulnerable and at-risk older adults.
