Work Schedule & Location

Schedule: Part‑time, 2 X 10 hours Day Shift. Every Saturday and Sunday, 8:00 AM–6:30 PM

Responsibilities

Assess patients' needs and coordinate the overall interdisciplinary plan of care from admission to discharge within Case Management departments.

Provide support and education regarding resources available to cope with emotional, financial and social needs that arise with a diagnosis.

Develop care plans for patients based on assessment findings and consideration of needs and preferences.

Assess complex emotional, psychological and physical issues to understand what the patient requires and how social work can provide support.

Advocate for patients' needs and ensure the medical system respects their rights and caters to their preferences.

Maintain ongoing dialog with leadership and care transition team members to ensure effective implementation and reevaluation of care plan.

Requirements

Licensure / Certification / Registration:

Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.

Certified Social Worker preferred.

Education:

Bachelor's degree in Social Work required.

Work Experience:

3 years of experience preferred.

Additional Preferences

Masters in Social Work required.

LCSW or LSW preferred.

Medical Social Services experience preferred.

Women's/Infant and Pediatric Experience Preferred.

Prefers working with perinatal, neonatal, and pediatric populations, including medically fragile infants and families in crisis.

Values a family‑centered approach that supports parents, caregivers, and multidisciplinary care teams.

Enjoys conducting psychosocial assessments, safety evaluations, and discharge planning for OB, NICU, and pediatric units.

Prefers strong collaboration with nursing, case management, physicians, lactation consultants, and child life specialists.

Enjoys supporting families through grief, adjustment, trauma, postpartum challenges, and complex social needs.

Values coordinating with CPS, early intervention services, community supports, and home‑visiting programs.

Prefers roles that emphasize advocacy, emotional support, and ensuring safe, stable discharge environments for infants and children.
