Responsibilities
Assess patients’ needs and coordinate the interdisciplinary plan of care from admission to discharge.
Provide support and education regarding resources available to cope with emotional, financial, and social needs that arise with a diagnosis.
Develop care plans for patients based on assessment findings and consideration of needs and preferences.
Assess complex emotional, psychological and physical issues to understand what the patient requires and how social work can provide support.
Advocate for patients' needs and ensure the medical system respects their rights and caters to their preferences.
Maintain ongoing dialogue with leadership and care transition team members to ensure effective implementation and reevaluation of the care plan.
Requirements
Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred; ACM also accepted.
Certified Social Worker preferred.
Education
Bachelor’s degree in Social Work required.
Work Experience
3 years of experience preferred.
Additional Preferences
Master’s in Social Work required.
LCSW or LSW preferred.
Medical Social Services experience preferred.
This is an acute inpatient position; 3 years of experience preferred.
Prefers working with patients who have frequent or high‑risk readmissions and complex psychosocial needs.
Values a fast‑paced environment focused on early intervention, root‑cause identification, and reducing avoidable readmissions.
Enjoys completing IAs, risk assessments, safety planning, and coordinating post‑discharge supports.
Prefers strong collaboration with CMs, nursing, providers, and community partners to address barriers to safe transitions.
Enjoys problem‑solving around social determinants of health, resource gaps, and care continuity challenges.
Values data‑informed work, targeted interventions, and opportunities to develop strategies to reduce recidivism.
Prefers roles offering autonomy, consistent communication, and a team approach to improving patient stability after discharge.
