Overview As the Social Services Director you will identify and provide for each resident’s social, emotional, and psychological needs, help their full potential develop during their stay, and assist in planning for discharge.
Responsibilities
Assist residents and their families with social, emotional, and family issues.
Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
Participate in resident care planning.
Participate in the development of discharge planning and prepare residents for transition.
Serve as a general source of information for residents and their families.
Qualifications
Bachelor’s or Master’s Degree in Social Work or Psychology.
3+ years of experience in Social Services, directly with individuals, preferably in a healthcare setting.
High level of professionalism and ability to maintain confidentiality.
