Step‑Down Care Navigator – full time, located in Virginia Beach, VA. Candidates must be available on weekends and holidays.
Responsibilities
Meet with all eligible patients to discuss benefits of outpatient services as part of their continuum of care treatment.
Communicate eligibility to key members of senior leadership and associated departments.
Collect and enter data specific to outpatient eligibility into Midas for trending report purposes.
Apply the step‑down playbook, implementing components as needed.
Assist with case management and discharge planning in the inpatient workflow (preferred).
Qualifications
High School Graduate or GED required; Bachelor’s degree preferred.
1‑3 years of experience in the psychiatric/mental health field.
Knowledge of addiction disease concept, chemical dependency treatment processes, 12‑step recovery programs, dual diagnosis, group dynamics, supervisory skills, and advanced oral and written communication skills.
Knowledge and adherence to HIPAA regulations regarding protected health information.
Performed other duties as assigned or required by this position.
