The Fargo VA Health Care System is recruiting for 1 full‑time Social Worker to serve as Housing and Urban Development (HUD)/VA Supportive Housing (VASH) – Health Care for Homeless Veterans (HCHV) Case Manager.

The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This approach empowers and equips people to take charge of their health and well‑being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America’s Veterans by providing exceptional health care that improves their health and well‑being.
The Social Worker is a part of the Fargo VA Health Care for Homeless Veterans (HCHV) team and reports to the Rural VASH Supervisor under the direction of the Homeless Programs Manager and the Social Work Executive. They will function as the Bismarck CBOC’s HUD/VASH Case Manager, operating in the Bismarck/Mandan and surrounding areas to include various parts of Western North Dakota, as feasible. This position will also serve as an outreach worker and strive to develop positive relationships with community providers. The incumbent is responsible for oversight of the region of the Bismarck HUD/VASH and HCHV programs.
Duties include, but are not limited to: administrative data collection and reporting; serving as the primary contact with local Public Housing Agencies (PHAs); and providing case‑management‑based services (directly or through referral) necessary to ensure that Veteran care is coordinated with the VA Clinic, community health or mental health providers, CBOC, and agencies that serve homeless populations. The incumbent will provide primary clinical psychosocial and case management services to eligible Veterans and their family members, meeting biological, psychological, social, and environmental needs. Veterans eligible for HUD/VASH and outreach efforts will cover a wide geographic area, and their physical and mental health needs may be complex, requiring a high degree of oversight and creative problem solving. Services are provided in healthcare settings and in the community. The outreach and HUD/VASH programs require continuous coordination with the community’s homeless providers and coalitions as well as with other local agencies. The program is a collaborative effort. The primary goal of the program is to move Veterans and their families out of chronic homelessness. A primary component of the program is VA case management services which are designed to improve Veteran’s physical and mental health, and to enhance their ability to remain stable, housed, and community‑integrated.
