Facilitates educational groups regarding nutrition, physical activity, smoking cessation, stress management, and medical needs for clients. Provides educational support to increase knowledge of clients and mental health providers regarding medical conditions. Plans and facilitates wellness activities.**Specialty Responsibilities:** **Standard Staffing Level Responsibilities:** Complies with all health and safety regulations and requirements.Performs other similar and related duties as required or directed.All responsibilities are essential job functions. **II. Position Qualifications:****License/Certification/Education:** **Experience/Skills:**1. Experience in the area of practice is necessary.3. Demonstrates proficiency in standard nursing practice including assessment, intervention, and evaluation.4. Demonstrates ability to communicate effectively and professionally with consumers, staff and providers.5. Demonstrates proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies.6. Strengths/Experience in Wellness oriented care plans including nutrition, physical activity, tobacco cessation, and stress management.Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description.
