Job Summary The Case Manager Generalist provides case management services to clients classified as low acuity. The case manager serves as the primary point of contact, coordinating services and care for assigned clients and handling a caseload of individuals who need a wide variety of human services resources. The role involves conducting comprehensive screenings, developing case plans, coordinating service linkages, attending case conferences, and acting as an advocate for clients in judicial, community, and administrative settings.
Duties & Responsibilities
Conduct comprehensive screenings and assessments with individuals or families to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information.
Assess barriers facing the clients and develop a case plan.
Coordinate individualized planning with clients to meet short‑ and long‑term needs.
Provide service linkages and support systems to ensure identified needs are met.
Coordinate case conferences with residents, consult with the service team, monitor progress of clients on the case plan, and assist clients in meeting agreed‑upon goals.
Act as an advocate for the client before judicial, community, social service, and administrative bodies as needed; facilitate support groups and client participation as appropriate.
Essential Duties
Assessment: Regular individual meetings to develop a detailed action plan, set goals, and find resources.
Coaching and Social Support: accompany participants to activities and services.
Coordinate Care and Case Management: connect participants to appropriate services.
Advocacy: represent participants before agencies and community settings.
Capacity Building: motivate participant progress.
Outreach and Engagement: meet participants and bring them into programs.
Other duties as required and may be subject to change.
Qualifications Requirements:
Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment, and education verifications.
Education:
HS/GED; completion of VOALA Case Management training within 6 months of hire.
Experience:
Minimum 1‑year general experience in case management or a similar social services role.
Or a Bachelor’s Degree in Social Services related field in lieu of experience.
Preferred Qualifications:
Bachelor’s Degree (Social Work, Social Services, public administration, public health or related field).
1 year of experience working with the target population.
HMIS navigation skills.
