Colorado Mental Health Hospital in Pueblo (CMHHIP) – Social Work / Counselor I – Full Time

CMHHIP is a 24-hour behavioral health acute care facility that provides a safe, secure, and therapeutic setting for patients with psychiatric and medical issues. The services include evaluation and treatment for patients who are severely mentally ill, with the goal of returning patients to their optimal level of functioning and assisting them in returning to the community as appropriate.

This posting may be used to fill multiple vacancies both full and part time. This recruitment is only open to current Colorado residents.

About the Position Under supervision of a Licensed Clinical Social Worker (LCSW), the Social Worker provides psychosocial therapies and individual, family, and group treatment to severely emotionally disturbed or cognitively impaired patients and their families. Duties include assessments, disposition planning, interagency communication, and ensuring continued appropriate level of services following discharge. Individualizes psychosocial therapy approaches and provides training and consultation to other professional staff.

Responsibilities

Completes psychosocial assessments by interviewing, collaborating, analyzing, and synthesizing social histories to inform treatment planning and continuity of care.

Provides required documentation; conducts annual psychosocial assessments for patients in hospital over 1 year when applicable.

Community Reintegration: coordinates discharge planning, assesses community resource needs, and coordinates appropriate placements and treatment programs; acts as liaison between hospital and community agencies; provides case management and referrals to community services; attends related meetings.

Treatments Planning: contributes to direction and formulation of treatment plans; collaborates with multidisciplinary teams to design interventions; ensures patient and family education needs are met; provides necessary documentation.

Treatment: provides individual, family, group therapy and systemic interventions respecting diversity; documents treatment activities.

Professional Development: stays current with theories and practices; collaborates to gather outcome data and evaluates treatment effectiveness; documents progress as required.

Other Duties as Assigned: performs additional duties as needed.

Minimum Qualifications

Four (4) years of relevant experience in an occupation related to the work assigned to this position, or

A combination of related education and/or relevant experience equal to four (4) years.

Preferred Qualifications

Masters in Social Work (MSW)

Knowledge of casework methods, human growth and behavior, and related treatment practices

Knowledge of interviewing skills and techniques; ability to express ideas effectively in writing and speech

Ability to exercise good judgment in evaluating situations and making decisions

Conditions of Employment

May work various shifts; must possess a current, unrestricted Colorado driver's license

Two-step TB screening is required of all new employees upon hire

Pre-employment background check required due to direct contact with vulnerable persons; certain charges/convictions may disqualify

No romantic or sexual relationships with patients; disclosure required on application

Pass CMHIP New Employee Orientation modules and competency assessments

Physical requirements for CTI and CPR; travel may be required

Annual influenza vaccination; other health and safety requirements

Former CDHS employees terminated for disciplinary reasons may have restricted eligibility
