Social Work/Counselor I - Benefits Specialist - CMHHIP Social Work Position within the Colorado Mental Health Hospital in Pueblo (CMHHIP). This role provides social work services within an interdisciplinary team to support discharge planning, treatment planning, and community reintegration for patients with psychiatric and related conditions.

Overview CMHHIP is a 24-hour behavioral health acute care facility offering evaluation and treatment for patients who are severely mentally ill. The social work/counselor focuses on psychosocial assessments, treatment planning, discharge planning, and coordination with community resources to ensure appropriate post-discharge services.

Responsibilities

Under the supervision of a Licensed Clinical Social Worker (LCSW), provide therapeutic interventions including psychosocial therapies, and individual, family, and group treatment.

Complete psychosocial assessments by interviewing and analyzing social histories to inform treatment and continuity of care; generate clinical summaries and recommendations.

Provide required documentation and, when applicable, annual psychosocial assessments for patients with extended hospital stays.

Coordinate discharge planning and assess patient needs related to community resources; serve as liaison between hospital and community agencies.

Facilitate case management by making referrals and maintaining relationships with community agencies, social services, court systems, and residential facilities.

Attend meetings with relevant agencies and participate in treatment planning with the multidisciplinary team.

Develop and review treatment plans; design interventions that address patient and family education and therapeutic needs.

Provide documentation for treatment processes and maintain accurate records.

Keep up to date with current theories and outcomes data to assess treatment effectiveness.

Perform other duties as assigned within the scope of the position.

Minimum Qualifications

Four (4) years of relevant experience in an occupation related to the work assigned to this position, or

A combination of related education and/or relevant experience equal to four (4) years.

Preferred Qualifications

Master’s in Social Work (MSW).

Knowledge of casework methods and behavioral health treatment practices.

Strong interviewing skills and ability to communicate effectively, both orally and in writing.

Conditions of Employment

May work various shifts; must possess a current, unrestricted Colorado driver's license.

TB screening, background checks, and clearance requirements as part of the employment process.

Compliance with safety, ethical, and professional standards; no inappropriate relationships with patients.

Ability to meet physical demands and participate in necessary training and orientation programs.
