As a Peer Support Specialist on the Homeless Outreach Team, you will build meaningful relationships that empower patients to achieve personal goals. Using lived experience, you will support patients with empathy, encouragement, and practical guidance.
Key Responsibilities
Collaborate with a multi-disciplinary team to support patients in achieving treatment goals.
Apply motivational interviewing skills to meet individuals at their recovery stage.
Engage individuals in identifying and working toward goals that build recovery capital.
Provide support that fosters independence, autonomy, and self-advocacy.
Assist individuals in accessing community resources that promote wellness and recovery.
Advocate to reduce stigma and promote resiliency.
Participate in treatment team meetings, individual and group supervision.
Communicate effectively with peers, managers, and support staff.
Maintain documentation in a timely manner.
Comply with confidentiality requirements (HIPAA, 42 CFR Part 2).
Provide culturally sensitive support.
Qualifications
Certified Peer Specialist or Recovery Coach (eligible for CRSW and Intentional Peer Support certificates).
Valid driver’s license and liability insurance in accordance with agency policy.
A minimum of a High School Diploma or GED.
Experience Experience working as a peer specialist in a mental health center setting and/or substance misuse services is highly recommended.
Education A minimum of a High School Diploma or GED is required.
