White Oak Medical Center seeks to hire an experienced Community Health Worker for our Care Navigation Department who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing.
As a Community Health Worker You Will
In collaboration with the Transitional Care RN, coordinate patient care with patient, family, and ancillary/community services to promote quality and cost‑efficient care for an assigned caseload.
Assist patients with enrollment and/or referring them to various health and social service programs, including meeting patients in the community to assist with connection when needed.
Assist patients to schedule medical appointments and connect them with transportation resources.
Provide follow‑up to patients by phone, home visits, and visits to other settings where patients can be found.
Conduct home visits to ensure that patients have what is required to recover safely at home, including safety checks (e.g., fall‑risk assessment) and preparation for follow‑up appointments with providers.
Qualifications Include
Bachelor of Science in health education, health promotion, or related field.
1–2 years of experience working in a community‑based setting with knowledge of local resources and system navigation.
Licensed driver who can operate a motor vehicle and maintain automobile insurance.
Active American Heart Association Basic Life Support (BLS) certification.
Work Schedule Monday – Friday, full‑time position. Hours: 9 am to 5:30 pm.
Pay Range $53,316.90 – $74,643.66
Under the Fair Labor Standards Act (FLSA) This position is classified as United States of America (Exempt).
