Position Summary:
A Certified Home Health Aide provides essential personal care and supportive services to patients in their homes under the direction of a Registered Nurse (RN) or other appropriately licensed professional. Duties include assisting with daily living activities, observing patient conditions, maintaining hygiene and safety, and promoting comfort and dignity of the patient.
Key Responsibilities: Assist patients with activities of daily living (ADLs) including:
Bathing, grooming, oral hygiene, toileting. Dressing, feeding, mobility (walking, transferring from bed, use of assistive devices). Assistance with range of motion exercises, repositioning, turning to prevent pressure ulcers.
Perform light housekeeping: meal preparation, cleaning, laundry, maintaining a safe and sanitary environment. Monitor basic vital signs (temperature, pulse, respiration), observe and report changes in patient condition to supervising RN or supervisor. Support patients' nutritional needs: help with feeding, ensuring fluid intake. Assist with transfers, ambulation, use of mobility devices (walker, cane, wheelchair), and ensure patient safety during those movements. Maintain accurate records of services performed and observations. Document in accordance with agency policies and regulatory requirements. Provide companionship, emotional support, and assist with social needs of patient and family when applicable. Follow care plans as detailed by RN / care team; adhere to all safety, infection control, and privacy (HIPAA) regulations. Preferred/Desired Qualifications:
Prior experience as a home health aide, hospice aide, or CNA. Experience with terminally-ill patients (for hospice aide roles). CPR certification. Good interpersonal skills, ability to work with patients, family members, and care teams. Working Conditions:
Work is conducted in patients' homes. Travel between clients may be required. Flexible or varied hours; may include evenings, weekends. Physical demands include lifting, bending, standing for prolonged periods, assistance with mobility.
