Key Responsibilities
- Provide comprehensive outpatient primary care to an assigned Veteran panel
- Practice within the PACT (team-based) model, collaborating with nursing, pharmacy, and mental health
- Lead evidence-based management of chronic and complex medical conditions
- Emphasize prevention, screening, and whole-health care planning
- Utilize VAs integrated EHR and specialty consult network to coordinate care
- Participate in quality improvement, patient safety, and population health initiative
