Our platform empowers provider groups and health systems to succeed in value‑based care contracts with payers by improving patient outcomes and reducing costs through better coordination, analytics, and disease management.
We are seeking a Patient Engagement Specialist to join our mission‑driven founding team. This role focuses on new patient acquisition, screening eligible patients, communicating with practice providers, and managing enrollment and ongoing patient engagement.
Responsibilities
Maintain daily on‑site presence at partner cardiology practices.
Support additional clinics remotely as needed.
Pre‑screen patients for eligibility and notify practice providers via program education.
Verify eligibility and gather feedback from practice providers.
Conduct enrollment onboarding sessions with patients.
Maintain documentation in the Karoo Health Kohere.AI system and partner cardiologist systems for tracking and reporting.
Drive enrollment by providing program overview and educating patients on services and benefits.
Support practice providers by enrolling patients and facilitating assignment to the virtual team.
Coordinate with the partner cardiology practice to provide insights into the patient’s care journey.
Interact with practice staff to provide visibility into patient admissions, readmissions, and ED visits.
Manage inbound calls from patients regarding enrollment and ongoing engagement.
Implement strategies to appropriately enroll patients.
Provide in‑services and presentations to practice partners about the Karoo Health program.
Communicate regularly with the interdisciplinary team to coordinate tasks and provide support.
Complete special projects, initiatives, and other duties as assigned.
Qualifications
High school graduate or equivalent.
Background as Community Health Worker, Medical Assistant, or LPN.
Cardiac experience preferred.
Minimum three years of related experience in health care and/or physician clinic/practice setting.
General understanding of enrollment and billing processes.
Strong interpersonal skills and ability to establish rapport quickly with providers and patients.
Comfortable in team dynamics and fit within clinic staff.
Confidence in explaining new programs and engaging with people.
Experience with team collaboration and care coordination with external sources.
Exceptional communication skills and active listening.
Strong written communication skills.
Comfortable with ambiguity in a fast‑paced culture; solution‑oriented mindset.
Ability to utilize data to make informed decisions.
