We deliver comfort, dignity, and support to patients and families in their homes, hospitals, nursing facilities, and at our Kaplan Family Hospice Residence. Our interdisciplinary team focuses on physical, emotional, and spiritual care, helping patients and families navigate life's most challenging moments.

What We Are Looking For:

Job Title: Hospice Aide Specialist - Community Clinical Care Team

Schedule: 2 shifts per week, every Saturday & Sunday

Shift:

8:30 am - 4:30 pm

About this role:

As a Hospice Aide Specialist, you'll make a meaningful difference in the lives of our patients and their families. This role provides direct, hands-on care in alignment with the patient's plan of care while supporting their comfort, dignity, and well-being. You'll work closely with our nursing staff and the interdisciplinary team to ensure each patient receives exceptional, personalized support.

What You'll Do: Assist patients with daily personal care needs including bathing, grooming, dressing, toileting, and mobility. Monitor and report patient observations and changes in condition to the RN. Help maintain a clean, safe, and healthy environment. Follow proper infection-control and safety procedures. Offer companionship through conversation, activities, and emotional support. Provide simple nursing support, vital sign checks, and emergency assistance as needed. Support caregivers by offering respite and responding promptly to patient/family needs. Work collaboratively as part of the hospice interdisciplinary care team. Requirements

Required Skills and Abilities:

Valid NYS Home Health Aide (HHA) certification High School diploma or equivalent Preferred Skills & Abilities

Minimum of one (1) year of experience in a hospital, nursing home, or similar clinical setting. Ability to read, understand, and follow written instructions, and accurately document care provided. Demonstrated initiative and the ability to work effectively with minimal direct supervision. Genuine empathy and sensitivity to the needs of individuals who are ill, frail, or functionally impaired. Understanding of the philosophy of hospice care and the unique needs of the terminally ill. Consistently demonstrate tact, patience, professionalism, and appropriate personal hygiene. Strong interpersonal and communication skills. Ability to remain composed and resilient in high-pressure or emotionally challenging situations. Excellent organizational and time-management skills.
