A Lead Navigator is an individual in recovery from AOD use and/or GMH who has at least 1 year of recovery and 90 days experience as a Navigator or Peer Support Specialist. The role involves outreach, engagement, and case management for homeless individuals, coordinating community-based services, and serving as a mentor to newer Navigators. Key Responsibilities

Conduct outreach and engagement to identify individuals with high acuity and vulnerability. Provide ongoing wrap‑around support services, including coordinated entry, housing referral, leasing, and discharge planning. Coordinate with multidisciplinary team members (clinical leads, counselors, nurses, etc.) to ensure comprehensive care. Act as a liaison between members and community providers to bridge gaps in care delivery. Mentor Navigator II staff, onboarding them and ensuring program compliance. Qualifications & Skills

High school diploma or GED (Associate’s degree in behavioral health preferred). 1–3 years of full‑time health‑care related experience or equivalent education/experience. Current AZ driver’s license and a 39‑month clean motor‑vehicle record. Behavioral Health Technician certification (BHT) as per A.A.C. R9‑10‑101.33 and CBI policies. Arizona Fingerprint Clearance card. Benefits
