Your Role as a Hospice RN Case Manager As an
RN Case Manager , you will play a
critical role
in providing
exceptional home health and hospice care . Your responsibilities will include:
Comprehensive Patient Assessments
- Evaluating the physical, social, and psychological needs of patients.
Collaborative Care Planning
- Engaging
patients, families, and interdisciplinary teams
to develop holistic care plans.
Patient-Centered Approach
- Providing compassionate support to
enhance comfort and quality of life .
Resource Coordination
- Managing patient care resources and referrals efficiently.
Accurate Documentation
- Maintaining meticulous
patient records
to ensure seamless care.
Skills & Qualifications
Current RN License in Nebraska
Home Health and Hospice experience preferred
Ability to
assist with patient transfers
using full range of movement
Strong verbal and written communication skills
Excellent time management and organizational abilities
Valid Driver's License and Reliable Transportation
Proof of Auto Insurance and Registration
Current CPR Certification (we can help obtain)
Negative TB Test (we can provide testing)
HEP B Record
