Overview
Case Manager (Bilingual English/Spanish) – Intermittent Hybrid/Remote – Bronx, NY
Position Summary:
As a case manager in our Health Home Department, you will develop an individualized care plan for patients with the end goal of placing them on a path to a healthy lifestyle with reduced need for hospitalizations. Health Home is a specialized service for Medicaid enrollees with two or more chronic health conditions (e.g., Asthma, Diabetes) or qualifying conditions such as HIV/AIDS or SMI.
Responsibilities
Conduct screening and assessments to develop a comprehensive and individualized plan of care.
Collaborate with co‑workers and other departments to help patients overcome obstacles through impactful care coordination (health care providers, mental health providers, and behavioral health providers).
Conduct home visits, meet with patients in health centers, conduct check‑in calls, and other means of contact. Enter case notes into electronic records.
Provide case management, patient education, advocacy, and referrals for services (e.g., housing, social services, benefits/entitlements, food programs).
Participate in interdisciplinary team meetings and quality improvement initiatives.
Qualifications
A Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or related field of study.
2+ years of experience in case management, community outreach, or providing direct care management services.
Excellent oral and written communication skills.
Comfort and flexibility with carrying out field work and home visits.
Understanding of electronic record keeping and treatment planning/case notes.
Bilingual Spanish/English, required.
