JOB SUMMARY:
The Outreach Specialist links adults and families to healthcare, public entitlements, nutritious food, affordable housing, and other needed social and community support.

- Outreach Specialist will meet with member of the community to offer support in housing, Food, rental arrears, and connection to behavioral health and Primary Care Services
- Staff will enroll all new program admissions in EHR CORE and conduct monthly documented service follow up on all program participants.
- Provide linkage to community services including medical, behavioral health, housing support, food assistance, entitlements, and other social services through communications with different programs
- Conduct outreach and engagement activities within East New York and Brownville Communities.
- Conduct evidence-based SDH, Primary Care, and Behavioral Health Screenings for all program participants.
- Provide and follow emergency response, if necessary, as outline by the company
- Ad hoc duties as assigned
ESSENTIAL KNOWLEDGE, SKILLS AND ABILITIES:

- Ability to work with clients, families, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences
- Demonstrated competency in written, verbal, and computational skills to present and document records by program standards.
- Ability to communicate effectively with staff, clients, families, and the public.
- Ability to understand and adhere to internal and external laws, rules, and policies.
- Ability to secure the cooperation of and work effectively with others.
- Knowledge of universal Health and Safety protocols
- Knowledge of Medicaid, Social Security, and other entitlements
- Excellent interpersonal skills.
QUALIFICATIONS AND EXPERIENCE:
High School Diploma with a minimum of two years of experience working with people with disabilities, human services, safety, and security or community base organizations or: Certified Peer Counselor. Bilingual Spanish preferred.
