Field-based role with regular field/home visits Coverage Area:
Brownsville, TX and surrounding cities Pay Range:
TBD / Based on experience Home Health Care Manager (RN) Responsibilities: Assess and manage care for high-risk, high-acuity long-term care members Develop, implement, and continuously evaluate individualized care/service plans Coordinate care among members, families/caregivers, providers, and community resources Monitor member status, changes in condition, and progress toward care plan goals Identify the need for higher levels of care or waiver eligibility when applicable Review member data, referrals, and intake assessments to improve quality and outcomes Educate members and caregivers on disease processes, benefits, services, and care options Serve as a member advocate and liaison between providers, families, and agencies Coordinate service authorizations, including meals, housing, transportation, and ADLs Conduct required home and site visits (typically monthly or more frequently as needed) Ensure accurate documentation and compliance with state and federal regulations Collaborate with leadership to support quality improvement and cost-effective care delivery Support onboarding, precepting, and coaching of new clinical hires as needed Home Health Care Manager (RN) Qualifications: Graduate of an accredited School of Nursing (Required) 4???6 years of RN experience (Required) Active RN license (State and/or Compact State Licensure) (Required) 4???6 years of home health and/or case management experience (Preferred) Bachelor of Science in Nursing (BSN) (Preferred) If you are interested in this
