Physical Therapist for Home Health PRN Agency in Warrensville Job Summary: We are seeking a skilled Physical Therapist to join our Home Health PRN Agency in Warrensville on an as-needed basis. The Physical Therapist will be responsible for evaluating patients' condition, creating and implementing treatment plans, and helping them improve their mobility and relieve pain.
Qualifications:
Bachelor's or Master's degree in Physical Therapy
Valid state licensure as a Physical Therapist
Proven work experience as a Physical Therapist
Knowledge of relevant therapy techniques and principles
Excellent interpersonal and communication skills
Ability to work independently and collaboratively with a team
Strong empathy and patience for working with patients
Responsibilities:
Perform initial and ongoing evaluations to determine patients' physical needs
Develop customized treatment plans based on patients' conditions and goals
Provide exercises and manual therapy to improve patients' mobility, relieve pain, and prevent disability
Educate patients and their families on at-home exercises and self-care techniques
Document and maintain accurate patient records
Collaborate with other healthcare professionals as needed to ensure comprehensive patient care
Working Conditions: This position is on an as-needed basis, requiring flexibility in scheduling. The Physical Therapist will primarily work in patients' homes, requiring travel to various locations in Warrensville.
