Position Overview The DCW provides personal care and related services in the client’s home, functioning under the direction, instruction and supervision of the Clinical Director and/or appropriate supervisor. The DCW receives scheduling direction from the Client Services Coordinator and/or supervisor.

Essential Job Functions

Assist with personal hygiene: bathing, shaving, grooming, dressing.

Maintain a healthy, safe environment.

Plan, prepare and feed nutritious meals; complete errands as instructed.

Assist with transfers and ambulation, with or without an assistive device.

Provide medication reminders.

Encourage independence per nursing care plan.

Promote mental alertness via engaging activities.

Offer simple emotional and psychological support to client and household members.

Establish trustful relationships with client and family.

Maintain confidentiality of client/family.

Prepare and submit visit reports, incorporating into clinical records.

Report changes in client’s mental or physical condition.

Report changes in home situation to the company.

Carry out assignments as instructed and report pertinent changes.

Perform routine housekeeping tasks to ensure safety and comfort.

Participate in in‑service education as required by policy and regulations.

Confirm weekly scheduling of visits with Supervisor/Director.

Notify the company of absences due to illness, emergency leave, vacation, bereavement or professional meetings that affect service.

Requirements Preferred: Completion of high school diploma or equivalent. Must possess basic math, reading, and writing skills and be able to speak, comprehend, read, and write English. Completion of a basic aide training program or Direct Care Worker test, satisfactory performance on training competency evaluation and/or exam, and ongoing annual re‑evaluation. Preferred: One year’s employment as a personal care aide, nurse’s aide or in another healthcare related field. Must have emotional and mental maturity, reliable transportation, completion of required health screening, at least two satisfactory references, valid driver’s license and acceptable MVR.

Physical Requirements

Read 12‑point or larger type.

Hear and speak in a manner understood by most people.

Sto op and bend.

Travel to client residences.

Safely lift, turn and transfer clients weighing up to 50 pounds.

Carry bundles up to 10 pounds up stairs.

Environment Function in a variety of environments that may involve allergens and other conditions.
