Position Overview The Direct Care Worker (DCW) provides personal care and/or related services under the direction of the Clinical Director and/or supervisor. The DCW receives scheduling from the Client Services Coordinator.
Essential Job Functions
Help client maintain personal hygiene (bathing, shaving, grooming, dressing)
Assist in maintaining a healthy, safe environment
Plan, prepare, and may feed client nutritious meals and complete errands when instructed
Assist client with transfers and ambulation with or without assistive device
Provide medication reminders
Encourage client to become as independent as possible per nursing care plan
Promote mental alertness through activities of interest
Give simple emotional and psychological support to client and household members
Establish relationship with client and family to build trust and confidence
Maintain client/family confidentiality at all times
Prepare a visit report and incorporate into clinical record weekly or as directed
Report any change in client’s mental or physical condition
Report any changes in home situation to the company
Perform assigned duties as instructed by supervisor and report pertinent changes
Execute routine housekeeping tasks appropriate for a safe, comfortable environment
Participate in in‑service education as required by company policy and state/federal regulations
Confirm weekly scheduling of visits with Supervisor/Director to coordinate with other personnel
Notify company of absences (illness, emergency leave, vacation, bereavement, professional meetings) affecting service
Requirements
Completion of high school diploma or equivalent (preferred)
Basic math, reading, writing skills
Proficient English speaking, reading, writing
Completion of a basic aide training program and/or Direct Care Worker test per state/federal requirements
Successful training competency evaluation and ongoing annual re‑evaluation
Preferred: one year’s employment as personal care aide, nurse’s aide, or other healthcare related field
Emotional and mental maturity for establishing and maintaining good relationships with client, family, and company personnel
Reliable transportation to assignments
All applicable health screening(s) completed
Minimum of two satisfactory references and background screenings before hire
Valid driver’s license and acceptable MVR
Physical Requirements
Read 12‑point or larger type
Hear and speak clearly
Stooped and bent posture acceptable
Travel to client residences
Safely lift, turn and transfer clients weighing up to 50 pounds
Carry bundles up to 10 pounds up stairs
Environment Work in a variety of environments that may involve exposure to allergens or other conditions.
Other Duties Additional duties may be assigned as needed; responsibilities may change with or without notice.
