Position Summary
Registered Nurse –Aluma Home Care The Registered Nurse (RN) in a non-medical home care setting is responsible for providing high-quality care to clients in their homes. This role includes conducting initial intake assessments, developing and updating individualized care plans, performing follow-up assessments, and supervising personal care attendants. Key Responsibilities
Conducts Home Health Aide Orientation
Reviews and certifies caregiver skills competency checklist. Infection Control review and certification. Reviews Care Plan with each aide for knowledge and understanding.
Initial Intake Assessment:
Conduct comprehensive assessments to evaluate clients’ health status and care needs. Collect medical histories, current medications, and other relevant information.
Care Plan Development:
Develop individualized care plans based on assessment findings, client preferences, and best practices. Collaborate with clients and families to set achievable goals and desired outcomes.
Care Plan Updates:
Regularly review and update care plans to reflect any changes in clients’ conditions or needs. Document all modifications accurately and promptly.
Follow-Up Assessments:
Perform ongoing assessments to monitor clients’ progress toward their care goals. Communicate findings effectively with clients, families, and the care team.
Medical Supervision of Home Health Aides:
Oversee and guide personal care attendants in the delivery of care. Provide training and support to ensure compliance with care plans and quality standards.
Qualifications
Current Registered Nurse (RN) license in Ohio Experience in home care or community health nursing is advantageous. Strong assessment, critical thinking, and communication skills. Ability to work independently and collaboratively within a team. Compassionate and patient-centered approach to care. Working Conditions
Primarily work in clients’ homes with occasional office meetings. Flexible scheduling to meet client needs. Conducts on-site Orientation. Performance Expectations
Maintain high standards of care and professionalism. Ensure effective communication with clients, families, and team members. Comply with all health and safety regulations and organizational policies.
