Community Health Worker – RCCC (Reentry & Community Coordination)

Company: HealthCare Access Maryland (HCAM) – nonprofit organization serving Maryland residents with health insurance and care coordination.

Pay Range Base pay: $23.00/hour (fixed).

Job Summary The RCCC Community Health Worker provides outreach, social support, care transition, and community resource information to soon‑release clients and their families. CHWs coordinate medical and social services over a minimum 60‑day period, maintaining a caseload of ~35 individuals. The CHW promotes access to services, schedules appointments, accompanies clients, educates on community resources, assists with benefits applications, coaches self‑management, conducts community visits, and ensures active medical insurance coverage.

Essential Duties and Responsibilities

Conduct social needs screenings and health risk assessments prior to clients’ release from custody.

Collaborate with State facilities, HCAM Benefits Specialists, Medical Discharge Planners, mental health teams, and DPSCS Reentry Coordinators.

Document assessments and services in electronic medical records.

Attend meetings at prison facilities, client homes/transitional houses, HCAM offices.

Use motivational interviewing techniques to engage and coordinate care.

Provide client‑centered services in a trauma‑informed approach.

Connect weekly for up to two months post‑release, and monthly thereafter for up to 12 months to provide ongoing assistance.

Consistently meet productivity goal and performance measures established for the role.

Additional Duties and Responsibilities

Empower clients to engage, re‑engage with community services.

Educate uninsured clients about health insurance programs; assist them in selecting an MCO and primary provider.

Refer to relevant programs in Baltimore City, Baltimore County, Anne Arundel County, or other Maryland districts.

Perform other duties as assigned.

Attend mandatory training courses and scheduled meetings.

Obtain DPSCS training to access Maryland State Institutions.

Follow HIPAA, confidentiality, and mandated reporting.

Report to appropriate authority representative; in cases of abuse, neglect, or imminent danger.

Work flexible non‑traditional hours as needed.

Qualifications Education, Experience, Certifications & Licensures

High school diploma or GED required; bachelor’s degree preferred in Human/Social Services.

Six months’ experience in a Community Health Worker role or related services.

Reliable transportation.

Active Maryland Community Health Worker Certification or willingness to obtain within 12 months of hire.

Knowledge, Skills & Abilities

Strong computer literacy: MS Word, Excel, Outlook, SharePoint 365.

Detail oriented for file and contract compliance.

Cultural and socioeconomic sensitivity.

Excellent oral and written interpersonal communication skills.

Efficient and deadline‑oriented.

Flexible and creative.

Compensation and Benefits Base pay: $23/hour.
