Tailored Care Manager
to join our multidisciplinary Tailored Care Management team covering
Bertie and Chowan counties .
Your Role As a
Tailored Care Manager , you will:
Evaluate client needs and develop personalized care plans
Coordinate services that address physical and mental health, social determinants, and other life challenges
Travel throughout Bertie, Chowan and surrounding counties to provide in‑community support and close gaps in care
Why Join Us Competitive salary:
$53,000 to $55,000
annually (based on experience and qualifications). Full‑time role with a Monday–Friday schedule (8 AM–5 PM) and rotating on‑call responsibilities.

Qualifications
Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area; Master’s preferred
Eligible for Qualified Mental Health Professional (QMHP) credentialing
Minimum 2 years of experience with mental health and/or substance use populations
Proficiency in Microsoft Office and Electronic Health Records (EHR)
Valid driver’s license, current auto insurance, and a clean driving record
Strong communication skills and ability to work independently
