Your Role in Our Mission As a
Tailored Care Manager , you’ll evaluate client needs, develop personalized care plans, and coordinate services addressing physical and mental health, social determinants, and life challenges. This role offers travel throughout Johnston and surrounding counties to close gaps in care.

What We’re Looking For We seek a compassionate, driven professional willing to provide services in various community settings and participate in rotating after‑hours support.
We Also Require
Bachelor’s degree in health, psychology, sociology, social work, nursing, or another relevant human services area (Master’s preferred)
Eligibility for Qualified Mental Health Professional (QMHP) credentialing
Minimum 2 years of experience with mental health and/or substance use populations
Proficiency in Microsoft Office and Electronic Health Records (EHR)
Valid driver’s license, current auto insurance, and clean driving record
Strong communication skills and ability to work independently
