# **DUTIES****SUMMARY**As a member of the REACH TEAMTM, the Community Services Care Coordinator supports children and families exposed to violence. Services to the family includes: care package drop off, care coordination services including screening, and referral to community agencies for children and families. In addition, this position participates in outreach and community engagement activities and trainings.**ESSENTIAL DUTIES**To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.* Engages community members and families with empathy and compassion following an incident of violence.* Identifies client’s needs and provides community referrals to client and others; coordinates* services with other providers.* Assists clinicians in providing appropriate services, including linkage, coaching and skill-building.* Develops and maintains effective communications with appropriate stakeholders.* Establishes and maintains rapport and effective working relationships with children and families* and works cooperatively within CII and community resources.* Ensures accurate and timely completion of outcomes and reports required by the program.* Develops and maintains knowledge of service standards and policies as stipulated by contract,* licensing, and/or other governing bodies.* Advocates on behalf of the client with other agencies, government programs and similar to help them receive needed services.* Participates in initial intake interviews to gather information and complete required initial paperwork.* Sets up and maintains related files and documentations ensuring compliance with policies.**ADDITIONAL DUTIES*** May conduct regular home visits to determine client needs and provides training, guidance and* support to families.* May participate as part of a multi-disciplinary team to develop and implement service plans.* May aid in the facilitation of group sessions.* Other duties and special projects as assigned.* One year of community-based direct service experience of working with families who experienced violence or trauma.* Bachelor’s degree in a human service industry; or four (4) years’ experience directly working with severely emotionally disturbed (SED) children and their families under the direct oversight of contracted services by either the Department of Mental Health (DMH) or Department of Children and Family Services (DCFS).* Possesses a valid driver's license and state-required auto insurance.* Required to travel extensively (> 50% of the time).* Spanish / English bilingual preferred.* Ability to handle confidential information.* Maintains patient privacy, including protecting the confidentiality, safeguarding the integrity and availability, and limiting the use and disclosure of patient Protected Health Information (PHI) to authorized individuals and entities.* Participates in, conducts, and sustains performance quality improvement activities according to designated role and responsibilities.* Sensitivity to service population’s cultural and socioeconomic characteristics.* Excellent verbal and written communication skills.* Punctual and dependable attendance.* Generous paid PTO, plus 10 paid holidays and CII is closed between Christmas and New Year* Excellent medical, dental, and vision insurance for eligible employees and qualified dependents* 403b Retirement Plan with employer contribution for eligible employees* Up to $4,500 in tuition reimbursement per calendar year* Eligible for the Public Service Loan Forgiveness program* Flexible Spending Account (FSA) & Health Savings Account (HSA)* Employer paid Life and AD&D Insurance* Voluntary Supplemental Insurance* Opportunity for growth & advancement* Professional development & continued training* Team building & bonding through company sponsored events & activities* Influenza immunization or declination* COVID-19, MMR and Tdap immunizations* Education verification* Reference check* Background fingerprint clearance (FBI, DOJ, CACI) conducted by the California Department of Social Services* Drug and alcohol screening* Tuberculosis screeningChildren’s Institute (CII) believes that all children and families have boundless potential to achieve emotional well-being and educational success, which build pathways to economic mobility and lifelong health.
